Uses of Itraconazole

  • Itraconazole is primarily used to treat both pulmonary and extrapulmonary forms of blastomycosis, histoplasmosis, including chronic cavitary pulmonary disease and disseminated nonmeningeal histoplasmosis, pulmonary and extrapulmonary aspergillosis, especially in patients intolerant of amphotericin B therapy, and onychomycosis [Toenail and fingernail involvement due to dermatophytes (tinea unguium)].

How Itraconazole works

  • Itraconazole falls under the triazole antifungal category.
  • It interferes with the synthesis of ergosterol, a crucial component of fungal cell membranes. This disruption weakens the fungal cell membrane, making it more susceptible to damage. By inhibiting ergosterol production, itraconazole inhibits fungal growth. Without a stable cell membrane, fungi struggle to maintain their structural integrity and reproduce. It further disrupts fungal cell function by affecting various enzymes and processes. This damages the fungal cell's ability to carry out essential functions, ultimately leading to its death.

Other Medications To Avoid With This Medicine 

  • Avoid taking antiarrhythmics (digoxin), anticonvulsants (carbamazepine), anti-HIV agents (indinavir), antineoplastics (busulfan), antipsychotics (pimozide), benzodiazepines (alprazolam), calcium channel blockers (nisoldipine), gastrointestinal motility agents (cisapride), HMG CoA-reductase inhibitors (atorvastatin), immunosuppressants (cyclosporine), oral hypoglycemics (repaglinide), opiate analgesics (fentanyl), polyene antifungals (amphotericin B), and anticonvulsants (carbamazepine).

Overdose And Missed Dose

  • If an overdose occurs and leads to severe side effects or worsening of any of the symptoms, it needs to be managed as an emergency in a hospital setting. Therefore, strict adherence to the prescribed dose is essential.
  • If you miss a dose of this medicine, take it as soon as you remember. But, if the next dose is near, skip the missed dose and resume the regular dosing regime.

  • Take this medicine with food to reduce gastrointestinal side effects and continue taking it as prescribed. If side effects worsen or persist, consult your doctor for a possible treatment plan adjustment.

  • It is unlikely to impair the mental alertness or coordination required while driving.

  • Caution is advised while giving this medicine to pregnant and breastfeeding women as it is not recommended for them.

  • Caution is advised while giving this medicine to older patients, patients having liver diseases, kidney diseases, cardiac dysrhythmias, hearing loss, neuropathy, congestive heart failure, peripheral oedema, and pulmonary oedema.

Special Dietary And Lifestyle Recommendation

  • Alcohol consumption should be avoided while taking this medicine as it may worsen the side effects.
  • Itraconazole should be taken 1 to 2 hours before or after taking the antacids.
  • Regularly change your socks and wash your feet. Avoid shoes that make your feet sweaty and hot.
  • Avoid walking barefoot in damp areas like changing rooms and gym showers to prevent fungal infections.
  • Resist the urge to scratch the affected skin area, which can spread the infection to other body parts.
  • Avoid sharing towels, combs, bedsheets, shoes, or socks with others.
  • Ensure you regularly wash your bed sheets and towels to keep them clean and free from potential fungal spores.
