Contraindications

• Pemetrexed is contraindicated in patients with a history of severe hypersensitivity reactions to the medicine or any of its components.

• Pemetrexed for Injection is not indicated for the treatment of patients with squamous cell, non-small cell lung cancer and Initial treatment, in combination with cisplatin, of patients with malignant pleural mesothelioma whose disease is unresectable or who are otherwise not candidates for curative surgery.

Other Medications to Avoid

• Pemetrexed may interact with NSAIDs or nonsteroidal anti-inflammatory medicines (e.g., Ibuprofen, Naproxen) and medicine that undergo tubular secretion (e.g., Probenecid, Cimetidine).

Overdose and Missed Dose

• Overdose of Pemetrexed may lead to symptoms like neutropenia, anaemia, thrombocytopenia, mucositis, and rash. In case of a suspected overdose, it is critical to inform your doctor immediately for appropriate management.

• If you forget to take this medicine, take it as soon as you remember. However, if it's nearly time for your next scheduled dose, skip the missed one and follow your usual dosing schedule. Do not double up your dose to compensate for the missed one.

Management of Side Effects

Most side effects of Pemetrexed are temporary and typically resolve once the course is completed or when the medication is discontinued. However, if any severe side effects occur or if symptoms worsen, consult your doctor.

• Mouth ulcers or sores can be managed by maintaining good oral hygiene.

Use in Driving and Operating Machinery

• Pemetrexed may cause fatigue, hence it is advised to avoid driving or operating heavy machinery until you know how this medicine affects you.

Use in Pregnancy and Lactation

• Pemetrexed is not recommended during pregnancy and breastfeeding unless recommended by your doctor. Consult your doctor before using this medicine during pregnancy and breastfeeding.

Caution in Other Conditions

• Patients with kidney disease should exercise caution while taking Pemetrexed. Dose adjustment is needed for patients with a creatinine clearance between 45 and 79 mL/min.

• Regularly monitor your renal function, as pemetrexed is primarily excreted through the kidneys.

• Monitor electrolyte levels, as pemetrexed treatment can affect fluid and electrolyte balance.
