Fosaprepitant

Uses of Fosaprepitant

  • The primary use of Fosaprepitant is to prevent nausea and vomiting associated with chemotherapy treatments for cancer. It falls under the category of neurokinin-1 (NK1) receptor antagonists.

Side effects of Fosaprepitant

  • Fatigue

  • Diarrhoea

  • Neutropenia

  • Asthenia

  • Anemia

How Fosaprepitant works

  • Fosaprepitant is an antiemetic medicine.

  • It works by being converted into its active form, aprepitant, in the body. Aprepitant blocks the action of a substance called substance P. Substance P is a chemical messenger in the brain that plays a key role in triggering nausea and vomiting. By blocking substance P, Fosaprepitant helps to prevent the feelings of nausea and vomiting often associated with chemotherapy treatments. In this way, it helps manage nausea and vomiting in patients undergoing highly emetogenic (highly likely to cause vomiting) or moderately emetogenic cancer chemotherapy.

Expert Advice

Dosage Forms Available

  • Fosaprepitant can be administered intravenously (powder which is reconstituted for injection).

Age and Dose Restrictions

  • This medicine is intended for use by adults and children aged 6 months and older.

Contraindications

  • Fosaprepitant is contraindicated in patients with a known hypersensitivity to any component of the product or aprepitant. It should not be used by patients who are currently taking pimozide due to potential increases in plasma concentrations of pimozide, which could lead to serious or life-threatening reactions.

Other Medications to Avoid

  • Fosaprepitant may interact with certain sedatives (such as midazolam) and steroids (such as dexamethasone and methylprednisolone), as well as cancer medicines (e.g., etoposide, vinorelbine).

Overdose and Missed Dose

  • An overdose of Fosaprepitant may lead to an increase in side effects such as severe nausea or vomiting. If you suspect an overdose, seek immediate medical attention.

  • If you miss a dose of Fosaprepitant, take it as soon as possible. However, if it is close to the time for your next dose, skip the missed dose and go back to your regular dosing schedule. Do not double doses.

Management of Side Effects

Most side effects are temporary and generally harmless and resolve when discontinuing this medicine. However, if you experience any severe side effects or worsening of any of the symptoms, please consult your doctor.

  • For side effects like fatigue and asthenia, try resting and limiting your activities.

  • If you experience diarrhoea, drink plenty of fluids to stay hydrated.

  • Fosaprepitant may cause drowsiness or dizziness in some individuals. Therefore, it is advised to avoid driving or operating heavy machinery until you know how this medicine affects you.

Use in Pregnancy and Lactation

Therefore, a decision should be made whether to discontinue breastfeeding or to discontinue using this medicine, taking into account the importance of the medicine to the mother.

Caution in Other Conditions

  • Fosaprepitant may affect the International Normalised Ratio (INR) in patients on chronic warfarin therapy. Regular monitoring is advised for these patients.

  • Patients with severe hypersensitivity reactions should exercise caution when using this medicine.

Special Dietary and Lifestyle Recommendations

  • Hydration is crucial as chemotherapy and associated medications can cause dehydration.

  • Stress management techniques like meditation, yoga or mindfulness can help manage emotional stress associated with chemotherapy.
