Uses of Dalteparin

  • Dalteparin primarily acts as an anticoagulant, falling under the group of medicines known as low molecular weight heparins (LMWH).

  • It has a range of applications including preventing and treating deep vein thrombosis (DVT) and pulmonary embolism (PE), preventing clotting in patients undergoing haemodialysis, managing unstable angina and non-Q-wave myocardial infarction (heart attack), preventing DVT in patients undergoing abdominal or hip replacement surgery, and extending the treatment of symptomatic venous thromboembolism (VTE) in patients with cancer.

How Dalteparin works

  • Dalteparin is categorised as a low molecular weight heparin.

  • It works by binding to a natural protein called antithrombin in the blood. This combination of Dalteparin and antithrombin then potently inhibits two other proteins, Factor Xa and thrombin (Factor IIa). These proteins play pivotal roles in the formation and growth of blood clots. By blocking the activity of Factor Xa and thrombin, Dalteparin prevents the conversion of prothrombin to thrombin and fibrinogen to fibrin, two processes that are crucial for blood clot formation. The net result is that Dalteparin interrupts the blood clotting cascade, thereby preventing the formation of new blood clots and the growth of existing ones.

  • Dalteparin is contraindicated in patients with known hypersensitivity to dalteparin, heparin, pork products, or any component of the formulation. It should not be used in patients with active major bleeding or in patients with thrombocytopenia associated with a positive in vitro test for antiplatelet antibodies after exposure to dalteparin.

  • Dalteparin can interact with oral anticoagulants (such as warfarin), platelet inhibitors (like aspirin, NSAIDs, dipyridamole) and thrombolytics (including alteplase, and streptokinase). It may also have significant interactions with other medications affecting hemostasis such as valproic acid, dextran, and hydroxychloroquine. Always consult your doctor before starting any new medications while on Dalteparin therapy.

Overdose and missed dose

  • Overdose of Dalteparin can lead to an increased risk of bleeding and possible haemorrhage. If you suspect an overdose, please contact your doctor immediately.

  • If you miss a dose of Dalteparin, administer it as soon as you remember. However, if it is close to your next scheduled dose, skip the missed dose and continue with your regular schedule. Do not double up on the dose to make up for the missed one.

Management of side effects

  • Most side effects of Dalteparin are temporary and generally harmless and resolve when discontinuing this medicine. However, if you experience any severe side effects or worsening of any of the symptoms, please consult your doctor.

  • To manage hematomas at the injection site, apply a cold compress to reduce swelling.

  • For nausea, try taking Dalteparin with food or milk.

Use in driving and operating machinery

  • Dalteparin does not usually affect your ability to drive or operate heavy machinery. However, if you feel dizzy or unwell after administration, avoid these activities until you feel better.

Use in pregnancy and lactation

  • Dalteparin falls into Pregnancy Category B, so it should be used during pregnancy only if clearly needed. It is not known to be secreted in breast milk in significant amounts. Always consult your doctor before using this medicine during pregnancy or breastfeeding.

Caution in other conditions

  • Dalteparin should be used with caution in patients with severe renal or hepatic impairment. Those with an increased risk of haemorrhage, severe uncontrolled hypertension or a history of gastrointestinal ulceration should also exercise caution when using this medicine.

  • Maintain a diet low in Vitamin K, as it can interfere with the effectiveness of anticoagulants like Dalteparin.

  • Avoid activities with a high risk of injury to prevent possible bleeding episodes due to anticoagulant therapy.
